It's never been easier to order the products you need from Midwest Veterinary Supply. Midwest now offers the convenience of ordering online directly from our secure JAT Rx Online Store.

1. Log in at www.midwestvet.net.

2. Click the "Shop JAT Rx" tab at the top right corner of the web page. You'll be taken to our online pharmacy.

3. Select a product, then select or create a pet owner and a corresponding pet(s).

4. Choose a quantity for the selected product and enter directions for its use.

5. Check out by agreeing to payment terms and authorizing the order.

Order by E-mail

Midwest Veterinary Supply now offers the ease and reliability of the order form you're used to, presented in electronic form.

1. Log in at www.midwestvet.net

2. Click the E-Rx Ordering Instructions for guidance regarding your Auto Signature set up.

3. Mouse over the Pharmacy Services link at the top of the website (do not click) and select "JAT Rx Order Form" from the drop down menu that appears below Pharmacy Services.

4. Fill out the form completely by typing into each field. All highlighted fields are required in order for the form to be submitted.

5. Save the completed form to your computer by using your computer's "File> Save As" command. (This can generally be located in the upper left hand corner of your open browser window.)

6. E-mail the form to the Pharmacy at This e-mail address is being protected from spambots. You need JavaScript enabled to view it .

Order by Fax

Midwest Veterinary Supply continues to offer our order form in PDF format for quick download, and offline use.

1. Log in at www.midwestvet.net

2. Mouse over the Pharmacy Services link at the top of the website (do not click) and select "JAT Rx Order Form" from the drop down menu that appears below Pharmacy Services.

3. Print the order form.

4. Fill out the form completely. All fields must be filled in order for the form to be accepted.

5. Please double check all fields on your form are filled out correctly and completely.

6. Fax your completed form to the Pharmacy at 877-490-3576.
